What You’ll Need to Complete This Form
Before you begin, please have the following documents and information ready to upload:
W-9 Form (current year, signed).
Trade License(s) – copy of your Michigan Builder, Electrical, Mechanical, or Plumbing license as applicable.
Certificate(s) of Insurance showing:
General Liability coverage ($1M per occurrence / $2M aggregate).
Auto Liability coverage ($1M).
Workers’ Compensation (or exemption affidavit if you are a sole proprietor).
BHI listed as Additional Insured with waiver of subrogation and 30-day cancellation notice.
ACH/Banking Information (for payments — voided check or ACH form).
Company & Contact Information (legal business name, mailing address, phone, email, emergency contact).
Safety/Certifications (EPA Lead-Safe, OSHA, or other applicable credentials if available).
